Care Coordinator

Israel joined our team in 2021 and has provided services in various roles. She started working as a Care Coordinator serving adults in the community who are experiencing severe and persistent mental illness. She transferred to work as a Case Manager for our ASCENT Program, and now is a Care Coordinator. He has developed several supportive counseling and intensive case management skills related to coordination of care. He is now proudly pursuing a Master in Social Work. Furthermore, he is passionate about serving the communities he is a part of, which include but is not limited to the queer/trans/LGBTQ/immigrant and Spanish-speaking communities. For fun, Israel likes to walk or jog in groups, play video games, and practice yoga.
