How to start care for yourself or a loved one

Finding high-quality mental health and substance use services is often harder than it should be. At Clinica Family Health & Wellness, our mission is to eliminate these barriers and make holistic physical, mental, and dental care more accessible for all. Begin your wellness journey with these 3 simple steps.

Step 1: Contact 

First, we need to collect basic information about you, your insurance status, income, and general state of health. Begin registration by calling our main phone number (303-443-8500) - or by downloading the registration form below. Once you complete and submit this form, we will reach out to schedule your assessment.

Step 2: Assess 

During your comprehensive assessment, a member of our team will spend some time with you to better understand your individual needs, concerns, and goals. These assessments can take up to 2 hours and be done virtually or in person.

Step 3: Match 

Once we have reviewed your assessment results, our experts will develop your custom treatment plan, recommend appropriate services, and match you with your treatment team to begin care.

Pre-appointment paperwork

We know paperwork can be a pain, but this information helps us provide quick and customized help for you, a loved one, or a patient. It can also help you better understand what to expect when receiving care from Clinica Family Health & Wellness. Please review and complete these forms at home before your appointment to shorten the registration process and begin care more quickly.

 

You can submit your completed forms in a couple of ways:

  1. Print. Print and bring them with you during your Comprehensive Assessment. (Call our main phone number at (303) 443-8500 to schedule, if you haven’t already.)
  2. Email. Download, complete, and send the documents to admissions@mhpcolorado.org.

Understanding cost and insurance

We believe cost should never be a barrier to getting the help you or your loved one need. For clients insured by Medicaid/Medicare, most of our services are completely free of cost. We also accept most private (commercial) insurance providers. If you are uninsured, underinsured, or our services aren’t covered by your insurance, we offer financial assistance to those who are eligible.

Get client-centered care in-person or online 

By offering in-person mental health services at locations in Boulder, Broomfield, Lafayette, Longmont, and Nederland, we aim to improve access by delivering expert care directly to the communities we serve. We also provide secure teletherapy services, allowing clients to get support virtually.
