SOCIAL DRIVERS OF HEALTH
Housing Services
OVERVIEW
Home plays a key role in happiness and health
Housing security is an important - yet often overlooked - element to a person’s overall health and wellness. The environment where we live has a profound impact in shaping our lived experience and sense of belonging. Data shows that housing insecurity leads to worse health outcomes and negatively affects mental health in many ways – including increased rates of stress, anxiety, and depression. What’s more, being unhoused can make it extremely challenging for individuals and families to access the essential health services they need to thrive.
At Clinica Family Health & Wellness, we understand the vital role housing plays in the health and wellness of our clients and patients, which is why we provide direct housing support services and resources.
No cost to clients
Our housing services are provided at no additional cost to our clients. These services are paid for by Medicaid, grant funding, generous donors, and other financial sources.
Team-based support
Each client is assisted by a team of experts ranging from therapists and peer support specialists, to case managers and housing specialists.
Client centered consultation
Our goal is to provide long-term solutions to homelessness and housing insecurity by helping clients find the support they need – whether that is applying for affordable housing, getting on a waitlist, securing a housing voucher, or finding a shelter. Each person’s story matters, and we provide customized support based on the unique challenges you face.
SERVICES
Building the foundations for health and wellness
Finding housing that is affordable, safe, and secure can be difficult and discouraging. Our housing team helps clients and patients navigate these challenges and unlock the benefits and resources available to them.
Meeting people where they are
We partner with local housing programs and shelters throughout our region – including All Roads Shelter (formerly Boulder Shelter for the Homeless) - to get individuals and families connected to the resources available to them. We also have therapists and providers available to provide direct support to people who are staying at the shelter and have trouble commuting to our locations.
Guiding clients to housing security
We help clients and patients navigate local resources and programs to find the financial assistance they need to secure housing, whether they are at risk of experiencing homelessness or unhoused already. This includes helping individuals apply for affordable housing.
Helping clients stay housed
We offer comprehensive consultations and referrals to help clients and patients overcome short-term housing challenges – including finding and keeping stable housing. We know what resources are available and can help you find the support you need to remain housed.
Securing affordable housing for you and your family
We manage over 500 housing vouchers for our clients – including for the Housing Choice Voucher (HCV) program, the Colorado State Housing Voucher (SHV) program, The Continuum of Care Permanent Supportive Housing (CoC PSH) program, and home ownership vouchers. By partnering with these and other local and state programs, we can help secure project-based and tenant-based rental assistance.
